Hair transplantation - a few words:
It is a known fact that 7 out of 10 men and 1 out of 5 women lose hair at a young age, this makes them lose confidence and makes them feel older. Once hair loss starts, they then are confused as to what need to be done, in a market flooded with lot of unapproved medications they invariably fall for the wrong medications, lose a lot of money, trying to grow hair, while answer lies in a simple solution of hair transplant surgery.

Hair loss needs to be graded and the severe ones benefit only from a transplant surgery. Unfortunately there are a lot of misinformation regarding this surgery in India . What is exiciting for my patients is with recent advances in administering local anaesthesia, microscopic dissection of hair follicles etc, hair transplant can be made as natural as possible undetectable to others. I have now started robotic follicular unit hair extraction as an extension, this procedure works well when the follicular units required is less. Causes of hair loss
Rather than going through the entire list of hair loss causes which people find it difficult to understand, there are a lot of diseases that can induce hair loss, including thyroid disease, auto immunity, fever, medications, dieting, stress etc. Aging can cause hair loss which can affect the entire scalp with thinning being detected after the age of 50 years.
The most commonest type of hair loss is androgenetic alopecia which affects males and females, with hair loss affecting the upper scalp, all though a lot of factors are blamed for this like using helmets, shampoos, poor circulation, none of this is true, and our Indian tendency of shaving the hair does not make hair growth better. Male pattern hair loss is a combination of faulty genes and hormones (dihydro testosterone-DHT) the genes are inherited from the genes are inherited from parents, and hair loss starts after puberty.The classical case of androgenetic baldness starts with thinning of hairs followed by eventual loss of hair itself, hairs on the scalp that is genetically affected by DHT under go this procedure. This can occur as a continous process or episodes of rapid hair loss, and it usually affects hair at the top of scalp, hairs at the sides and back are not affected by DHT and they form the donor area for hair transplant surgery. The best part of hair loss is that it can be treated when it is in the early stages with a tablet called finasteride and minoxidil lotions and more advanced cases with hair transplant surgery. If people understand this issue then it makes their hair loss problem a lot easier to handle. Lasers do not work on androgenetic pattern hair loss.

Surgery

Follicular unit hair transplant can deliver natural life long hair, the reason being hairs are normally present in the form of follicles in the scalp, which may be one, two or three. when we do a hair transplant we maintain the integrity of these follicles, as they are cut from the donor strip, under high powered microscopes, hair growth has been proven to deliver better results when intact follicles are transplanted, where as dividing a follicular unit can hamper hair growth, hence it is always better to choose a doctor who uses microscopic dissection for hair follicles, since this takes in a lot of experience to do so not many hair transplant surgeons opt for this.

Follicular unit hair transplant involves removing donor hair from the back portion of the scalp , under local anaesthesia and sedation, cutting individual hair follicles under the operating microscope and then making tiny cuts less than 1 mm in size to transfer these follicles into bald areas of scalp.

The use of follicular units result in creation of natural, feathered hairlines with out the abrupt ending of hairlines caused by older techniques. The requirement of number of grafts vary it may be as less as 500 to as much as 3000 plus depending upon the severity of hair loss, patients require between 1-2 sittings depending upon the extent of hair loss, to achieve satisfactory density (what we mean by density is the number of follicles I one square centimeter of skin). We have seen in our practice in my clinic an average density of 60 plus follicles / square centimeter in the donor region, Following transplantation small crusts develop which will shed of subsequently, there is no bandaging involved, grafted hair commences growth in 6-15 weeks and grow at a rate of 1 cm a month. The success rate of transplanted hair is very high and they do not get affected by DHT due to their generic immunity from DHT hormone. To achieve natural results the experience of doctor counts because it needs artistic ability to achive placement and angulation of grafts to promote natural results, hence patients should look at the background of doctors before commiting to hair transplant surgery. Follicular unit hair transplant surgery also is useful for covering up scalp scarring from trauma, burns or older surgeries on scalp.

Robotic Follicular unit extraction
We have pioneered robotic follicular unit extraction technique in India Where in a robotic device extracts individual hair follicles, from the back portion of scalp, up to 700-1000 follicles can be harvested in a single sitting and transplanted.

The advantages of follicular unit extraction are
Avoiding the need to suture the donor region, a wider donor region is available for hair extraction, post operative recovery is a lot better and faster compared to conventional hair transplant surgery, however we now use this technique to treat clients whose requirement is less than 1000 follicles, or in clients who refuse the traditional hair transplant surgery. The use of robotic follicular unit extraction has made hair transplant easier in patients in whom the requirement is small, this device cuts and removes the hair follicle at the same time, hence the time required to extract large number of hair follicles is less, compared to punches which other hair transplant surgeons use.

Is hair transplantation successful?
Yes! Hair transplantation has been continually refined over the last 40 years and Australia has a proud record as a leader in developing and refining advances in this field. In experienced hands, approximately 90% of the transplanted hairs will grow in their new position, permanently. Hair transplantation does not achieve your original hair density but generally provides good to excellent coverage.

Is hair transplantation safe?
Yes. Hair transplantation has a justified reputation as being a very safe solution. Only a few patients experience minor side-effects such as initial swelling and temporary numbness in the transplanted area. Minor bruising is very uncommon. Complications are very unusual and may include minor infection at the graft site which easily responds to antibiotics.

Is the procedure painful?
No. The procedure is carried out under local anesthetic and mild sedation (which makes you drowsy but awake) so that no discomfort is felt during the procedure. Mild analgesics are provided afterwards to relieve any minor discomfort felt over the 24-48 hours following the procedure. Ice packs may be used for the first 48 hours after the procedure to minimise any swelling that may occur.

Are there any scars?
A minor scar is created in the lower scalp donor area after removal of the donor skin. This area is stitched closed with fine dissolving sutures and a new closure technique encourages hair to grow through the scar. In any event the surrounding hair covers this narrow scar easily. Even your hairdresser will have trouble finding it! The graft sites in the upper scalp heal with almost no scarring which may rarely be barely visible upon close inspection if seen at all. In any case it is effectively covered by the transplanted hairs.

How many procedures are required?
This depends on the size of the area to be treated and the density required. A higher graft number per session will reduce the number of procedures. It will be discussed with you at the initial consultation. It may be as little as two procedures.

When does the new hair appear?
After the procedure small scabs or crusts form on the grafts. These protect the grafts. After 7-10 days these will flake off by themselves or may be gently removed. At this
time, some of the transplanted hair seen above the scalp will initially be shed. This is normal. However the hair roots or follicles below the skin will produce new hairs after 8-12 weeks. Each graft generally produces between 1 to 4 hairs. At first only a few hairs are seen, but many hairs are now growing beneath the skin and they will appear gradually during the first six months. The results are at first gradual and subtle, but very certain. 8-12 months after the procedure, when the hair has had a chance to grow out, the results are generally very pleasing.

When should I start treatment?
Patients may have hair restoration surgery at any age. If your hair is thinning, it is better to start before you are completely bald so that you can use existing hair to camouflage the procedures. However, because hair loss tends to be both gradual and progressive, it is often unwise to start surgical treatment in a patient who is very young. Medical treatments such as Finasteride and Minoxidil may be offered to men with mild to moderate hair loss to help preserve the hair in the crown. Hair transplants can be used to fill-in the front hairline and thicken the front half of the scalp, and medical treatments can be used to maintain hair behind the transplants and possibly enhance the long-term.

Hair transplants are commonly performed to treat hair loss in men although women are also affected. Baldness is a common condition in men, and some women, which causes a great deal of distress in both sexes.
